This case report describes the evaluation and management of a 21-year-old male patient with compound myopic astigmatism. This refractive defect significantly affects vision, impacting daily activities such as studying and prolonged use of electronic devices. The patient presented with symptoms of blurred vision, eye fatigue, headaches and light sensitivity associated with outdated optical correction and prolonged visual strain. The initial evaluation included a detailed anamnesis, medical history analysis, and optometric tests such as objective and subjective refraction, corneal topography and binocular evaluation. The results showed more severe bilateral astigmatism in the right eye and latent esophoria, attributable to lack of adequate correction and ocular misalignments. Treatment included spherocylindrical lenses with anti-reflective treatment to improve visual acuity and reduce visual stress. The practice of ergonomic habits, regular visual breaks and the implementation of visual therapies to manage esophoria were also recommended. In addition, a biannual follow-up was suggested to monitor the evolution and adjust the correction according to the patient's needs. In conclusion, this case highlights the importance of a comprehensive optometric evaluation and education in visual health to prevent the progression of refractive defects, with appropriate intervention, it is possible to significantly improve the quality of life and academic performance of young patients.es_ES
