dc.description | The present clinical case is focused on a 48-year-old diabetic patient diagnosed with type II diabetes mellitus two years ago, who attends the Baba Basic Hospital through the emergency area in the company of his son, who reported a clinical picture of approximately 3 days of evolution accompanied by hyperthermia 39 ºC, blurred vision, frequency, polydipsia and paresthesia. He also presented a septic point in the hemodialysis catheter and ulcer in the left lower limb (foot - heel) that urgently required surgical cleaning. Constants were taken; blood pressure: 132/74 mmHg, heart rate: 94 l/min, respiratory rate: 19 r/min, axillary temperature: 39 ºC, oxygen saturation 98%, weight 52 Kg and height 165. A glycolysis test was performed, resulting in 250mg/dl (13.9mmol/L). The patient reported that he did not take Metformin for three days. 14 units of insulin were administered and peripherally channeled into the right upper limb for medication administration. The doctor ordered urinalysis, hematology, blood chemistry and ultrasound examinations of the foot, to clearly appreciate the depth of the ulcer. A physical assessment was performed using the Marjorie Gordon patterns in order to identify the altered patterns that were compromising their health and well-being. Surgical cleaning was performed on the left foot. The Nursing Care Process was elaborated, using the Nanda, Nic and Noc taxonomies as tools, to meet the proposed objectives. | es_ES |

dc.description.abstract | El presente caso clínico está enfocado en paciente diabético de 48 años de edad con diagnostico de diabetes mellitus tipo II hace dos años, que acude al Hospital Básico de Baba por el área de emergencia en compañía de su hijo, quien refirió un cuadro clínico de aproximadamente 3 días de evolución acompañado de hipertermia 39 ºC, visión borrosa, polaquiuria, polidipsia y parestesia. También presentó un punto séptico en el catéter de hemodiálisis y úlcera en miembro inferior izquierdo (pie – talón) que ameritaba de urgencia una limpieza quirúrgica. Se procedió a la toma de constantes; presión arterial: 132/74 mmHg, frecuencia cardiaca: 94lxmin, frecuencia respiratoria: 19rxmin, temperatura axilar: 39 ºC, saturación de oxigeno 98%, peso 52 Kg y talla 165. Se realizó prueba de glucolisis, dando como resultado 250mg/dl (13,9 mmol/L). El paciente refirió que no se tomó la Metformina durante tres días. Se administra 14 unidades de insulina y se canalizó de vía periférica en miembro superior derecho para la administración de medicamentos. El médico ordenó realizar exámenes de uroanálisis, hematológicos, química sanguínea y ecografía del pie, para apreciar con claridad la profundidad de la úlcera. Se realizó valoración física empleando los patrones de Marjorie Gordon con el objetivo de identificar los patrones alterados que estaban comprometiendo su salud y bienestar. Se realizó limpieza quirúrgica en pie izquierdo. Se procedió a la elaboración del Proceso de Atención de Enfermería, utilizando como herramientas las taxonomías Nanda, Nic y Noc, para cumplir los objetivos propuestos. | es_ES |
