dc.descriptionPneumonia is an infectious and inflammatory process caused by various microorganisms that affect the lung parenchyma, filling the air sacs with pus or fluid, preventing proper gas exchange, one of its classifications is community-acquired pneumonia which is obtained outside the hospital environment, The annual incidence of CAP morbidity in adults fluctuates between 1.07 and 1.2 cases per 1,000 people per year. In Ecuador according to the INEC year 2016, CAP in the adult population ranked fifth (5.32%) within the top 10 causes of mortality. Community-acquired pneumonia occurs when the defense system is overwhelmed by a combination of factors such as a very large or very virulent bacterial inoculum or a deficiency in defense mechanisms. The treatment of CAP is fundamentally empirical, especially in the early stages. In this case, we will treat an adult smoker who arrived at the emergency area with high fever, cough and strong retrosternal pain of 5 days of evolution, a physical and complementary examination was performed, which showed us that it was community-acquired pneumonia by Streptococcus pneumoniae.es_ES

dc.description.abstractLa neumonía es un proceso infeccioso e inflamatorio causado por diversos microrganismos que afecta el parénquima pulmonar, llenando los sacos aéreos de pus o líquido impidiendo el correcto intercambio gaseoso, una de sus clasificaciones es la neumonía adquirida en la comunidad la cual se obtiene por fuera del ambiente hospitalario. La incidencia anual de morbilidad de la NAC en el adulto fluctúa entre el 1.07 y los 1.2 casos por cada 1.000 personas al año. En el Ecuador según el INEC año 2016, la NAC en la población adulta se ubicó en el quinto lugar (5,32%) dentro de las 10 primeras causas de mortalidad. La neumonía adquirida en la comunidad se presenta cuando el sistema de defensa es sobrepasado por una combinación de factores tales como un inóculo bacteriano muy grande o muy virulento o una deficiencia de los mecanismos de defensa. El tratamiento de la NAC es fundamentalmente empírico, especialmente en los primeros momentos. En este caso trataremos con un paciente adulto fumador que llego al área de emergencia con fiebre alta, tos y fuerte dolor retroesternal de 5 días de evolución, se le realizó el examen físico y complementario, los cuales nos arrojaron que era neumonía adquirida en la comunidad por Streptococcus pneumoniae.es_ES
