dc.descriptionSpontaneous liver rupture was first described by Abercrombie in 1844. The formation of a hepatic hematoma with subsequent rupture is a rare complication of pre-eclampsia-eclampsia. 90% of tears occur in these hypertensive disorders, the majority in relation to HELLP syndrome, the most unusual complication, but the most severe of this syndrome. The rest are mostly traumatic tears or attributable to other infrequent causes. Therefore, the objective of this project is to analyze the clinical evolution of eclampsia and its complication of liver rupture in a 26-week pregnant woman and her subsequent obstetric behavior for the management of her clinical picture. We present the case of a multiparous pregnant woman of ± 26 weeks' gestation by first trimester ultrasound, who was referred from a private clinic with a diagnosis of Eclampsia. Upon arrival at the health home, she presented tonic-clonic seizures and upon hospitalization a positive COVID-19 test for IgG and IgM was reported. The diagnostic methodology is based on the analysis of the patient's medical history and previous risk factors such as a pathological history of pre-eclampsia in a previous pregnancy and obesity. Patient's clinical picture triggers a liver rupture with subsequent death.es_ES

dc.description.abstractLa rotura espontánea del hígado, fue descrita por primera vez por Abercrombie en 1844. La formación de un hematoma hepático con rotura posterior, es una complicación rara de la preeclampsia-eclampsia. El 90 % de las roturas ocurre en estos trastornos hipertensivos, la mayoría en relación con síndrome HELLP, complicación más inusual, pero la más severa de este síndrome. El resto son roturas traumáticas en su mayoría o atribuibles a otras causas que son infrecuentes. Por el cual, el objetivo de este proyecto es analizar la evolución clínica de la eclampsia y su complicación de ruptura hepática en una gestante de 26 semanas y su posterior conducta obstétrica para el manejo de su cuadro clínico. Se presenta el caso de una gestante multípara de ±26 semanas de gestación por ecografía de primer trimestre, que acude referida de clínica particular con diagnóstico de Eclampsia. Al llegar a la casa de salud, presenta convulsiones tónico-clónicas y a la hospitalización se reporta prueba de COVID-19 positivo para IgG e IgM. La metodología de diagnóstico se basa en el análisis de la historia clínica de la paciente y factores de riesgos anteriores como antecedentes patológicos de preeclampsia en gesta anterior y obesidad. Cuadro clínico de paciente desencadena en una ruptura hepática con posterior muerte.es_ES
