PPH is one of the most frequent causes of maternal morbidity and mortality worldwide and one of the second causes of maternal deaths in our country. The following clinical case is about a 42-year-old, multiparous patient who attends the GENERAL HOSPITAL '' MARTIN ICAZA '' to the gynecological-obstetric emergency area or labor, referring pain under the belly with a gestational age of approximately 40 weeks, several hours after admission, a simple eutocic delivery occurs, but excessive blood loss is observed, producing postpartum hemorrhage due to uterine atony, the red code is activated and proceeds to act according to the protocols of the Ministry of Public Health, 2 units of concentrated red blood cells, but it should be noted that the patient belongs to the religion of the `` JEHOVA'S WITNESS '', which absolutely refuses to receive a blood transfusion, thus making her clinical picture much more delicate and complicated. with the relatives (HUSBAND) who accepts and signs the informed consent accepting the transfusion.es_ES

dc.description.abstractLa atonía uterina es considerada como hemorragia postparto primaria o inmediata, ya que esta se puede presentar posterior al parto o dentro del transcurso de las siguientes 24 horas después del parto, ya que el 70% de las hemorragias postparto se producen por atonía uterina siendo así la HPP una de las causas más frecuentes de morbimortalidad materna a nivel mundial y una de las segunda causa de muertes maternas en nuestro país. El siguiente caso clínico se trata de una paciente de 42 años, multípara que acude al HOSPITAL GENERAL ‘’MARTIN ICAZA‘’ al área de emergencia gineco-obstétrica o labor de parto, refiriendo dolores bajo vientre con edad gestacional de aproximadamente 40 semanas, varias horas posterior a su ingreso se produce un parto eutócico simple, pero se constata una perdida sanguínea excesiva, produciéndose una Hemorragia postparto por atonía uterina, se activa la clave roja y se procede actuar según protocolos del Ministerio de Salud Pública, se requiere 2 unidades de glóbulos rojos concentrados, pero cabe recalcar que la paciente pertenece a la religión de los ‘’TESTIGO DE JEHOVA ‘’, la cual se niega en lo absoluto a recibir la transfusión sanguínea, convirtiendo así mucho más delicado y complicado su cuadro clínico se procede hablar con los familiares (ESPOSO) el cual acepta y firma el consentimiento informado aceptando la transfusión.es_ES
