This work presents the evolution and outcome of a seventy-five-year-old patient with idiopathic pulmonary fibrosis with a history of being a smoker and seeing presented respiratory failure who develops pulmonary infection by a Mycobacterium avium where his disease reactivates. The patient's symptoms were nonspecific and reaching the final diagnosis required several hospitalizations and examinations. This case is important because it encompasses the entire approach to the treatment of a lung infection by this pathogen and tells us step by step how to treat the patient until his recovery since we know that older adults are more vulnerable to these diseases, whether acquired ambulatory or nosocomial mind. The patient successfully recovered from his illness after long months of treatment and acquired therapies against the bacterium Mycobacterium avium subspecies hominissius, which he acquired in the hospital during his intervention a month ago due to respiratory failure, which reactivated his underlying disease as It is idiopathic pulmonary fibrosis, several interventions were carried out with different procedures since his disease was a little unsatisfactory for the patient since at the beginning he did not see improvement in his health, he continued to present discomfort in his respiratory treatment.es_ES
