Within the surgical history, a supracondylar amputation was recently performed by diabetic foot; allergies does not refer; He is admitted by emergency for presenting a recurrent clinical picture characterized by pain at the level of lower hemiabdomen accompanied by dysuria, polyakyria and tenesmus; to the physical examination: patient in stable general conditions, skin: phototype 3 with turgor and diminished elasticity, capillary filling less than 3 seconds., soft abdomen depressible, not painful to palpation hydro-air noises present does not require neurological organomegaly, vigil, coherent language and fluid, globally diminished muscle strength, Weight: 52kg, Size: 162cm CB: 24cm and TCP: 10mm; Their laboratory data reflected hyperglycemia and moderate anemia; Your medical diagnosis, E109 DIABETES MELLITUS INSULINODEPENDIENTE .; The food consumption that the patient is currently receiving, does not cover with his daily caloric complaints, it is worth mentioning that the diet will be modified, the diet where they include nutritional supplementation, normocaloric diet with characteristics, hyperproteic, hyposodic, normolipidic, normoglucidal, high in fiber, divided into 5 meals a day, where it is providing the amounts of macro and micronutrients that the patient needs, their medications include losartan 50 mg orally if blood pressure + 140/90 mmhg, enoxaparin 20 units vsc and simvastatin 1 tab every day.	es_ES
dc.description.abstract	La Diabetes Mellitus tipo 2, es una enfermedad con un alcance global y en crecimiento exponencial, que está afectando a la población mundial, con altas tasas de mortalidad debido a las complicaciones a largo plazo, afectando además las economías de los países y la calidad de vida de los pacientes afectados; El presente caso clínico se trata de adulto mayor de 77 años de edad, con antecedentes de hipertensión, diabetes mellitus tipo 2, dentro de los antecedentes quirúrgicos recientemente se le realizo una amputación supracondilea por pie diabético; alergias no refiere; Es ingresado por emergencia por presentar un cuadro clínico a repetición caracterizado por dolor a nivel de hemiabdomen inferior acompañado de disuria, poliaquiria y tenesmo; al examen físico: paciente en estables condiciones generales, piel: fototipo 3 con turgor y elasticidad disminuida, llenado capilar menor a 3 segundos., abdomen blando depresible, no doloroso a la palpación ruidos hidroaereos presentes no precisa organomegalia neurológica, vigil, lenguaje coherente y fluido, fuerza muscular globalmente disminuida, Peso:52kg, Talla: 162 cm CB: 24 cm y TCP: 10mm, reflejando al momento un IMC de 18.2kg/m2 bajo peso severo con deficiencia energética grado II; Sus exámenes de laboratorio reflejaron hiperglucemias y anemia moderada; Su diagnóstico médico, E109 DIABETES MELLITUS INSULINODEPENDIENTE.; El consumo alimentarios que actualmente está recibiendo el paciente, no cubre con sus requerimientos calóricos diarios, cabe mencionar que se va modificar, la dieta donde esta incluyan suplementación nutricional, dieta normocalorica con características, hiperproteicas, hiposodica, normolipidica, normoglucida, alta en fibra, fraccionada en 5 comidas al día, donde esta aporte las cantidades de macro y micronutrientes que el paciente necesita, sus medicamentos incluyen losartan 50 mg vía oral si la Tensión arterial + 140/90 mm hg ,enoxaparina 20 uds vsc y simvastatina 1 tab cada día.	es_ES
