dc.description | The present clinical case study is based on a 68-year-old male patient who was diagnosed with gastric cancer as well as his father and brother who died of this cause. He came to the clinic for presenting upper gastrointestinal bleeding expressed by repeated hematemesis. and hair with abundant blood loss of approximately one liter that is accompanied by abdominal pain, adynamia, pallor and weight loss. The patient is given a comprehensive nutritional assessment and is determined through the body mass index that presents low weight with mild energy deficiency and mild malnutrition. On the other hand, biochemical tests were performed in which his results reflect anemia due to decreased values of red blood cells. , hemoglobin and hematocrit and the decrease in albumin values is due to the fact that there is a mild depletion, when the physical examination shows generalized pallor, due to the advanced stage of the cancer, a feeding plan must be made by parenteral nutrition as long as the gastrointestinal tract not functional and implement the administration of iron and vitamin C to reverse the anemia, then to recover the gastrointestinal tract with the disappearance of the bleeding will continue with enteral nutrition, through monitoring and monitoring that is done to the patient his weight increased 2 kilos, whereby an improvement is seen in the patient, his biochemical tests were raised but anemia still exists, in a critical oncological patient the energy requirements are: proteins of 1.5 to 2.5 g / kg / day, carbohydrates of 3 to 5 g / kg / day and lipids 1 g / kg / day. | es_ES |
dc.description.abstract | El presente caso clínico de estudio se basa en un paciente de sexo masculino de 68 años de edad que fue diagnosticado con cáncer gástrico al igual que su padre y hermano fallecidos por esta causa, acude a la consulta por presentar hemorragia digestiva superior expresado por hematemesis repetidas y melena con perdida hemática abundante aproximadamente de un litro que se acompaña por dolor abdominal, adinamia, palidez y pérdida de peso. Se le realiza valoración integral nutricional al paciente y se determina a través del índice de masa corporal que presenta bajo peso con deficiencia energética leve y desnutrición leve, por otra parte se realizaron exámenes bioquímicos en los que sus resultados reflejan anemia por valores disminuidos de glóbulos rojos, hemoglobina y hematocrito y la disminución de valores de la albumina se debe a que existe depleción leve, al examen físico presenta palidez generalizada, debido a la etapa avanzada del cáncer se debe realizar un plan de alimentación mediante nutrición parenteral siempre y cuando el tracto gastrointestinal no este funcional e implementar la administración de hierro y vitamina C para revertir la anemia, luego al recuperar el tracto gastrointestinal con la desaparición del sangrado continuaremos con nutrición enteral, mediante el seguimiento y monitoreo que se le realiza al paciente su peso aumento 2 kilos, por lo cual se ve una mejoría en el paciente, sus exámenes bioquímicos se elevaron pero aún sigue existiendo anemia, en un paciente oncológico critico los requerimientos energéticos son: proteínas de 1,5 a 2,5 g/kg/día, carbohidratos de 3 a 5 g/kg/día y lípidos 1 g/kg/día. | es_ES |
