dc.descriptionThe present study is focused on the pathology of idiopathic thrombocytopenic purpura that affects the immune system and is characterized by platelet dysfunction causing increased bleeding due to its clotting time. Among the most predominant clinical manifestations are mucocutaneous hemorrhages and the appearance of petechiae in any part of the body. This disease is treatable and with appropriate treatment the patient can have a full recovery. However, the following clinical case study is applied to a 19-year-old female patient at 22 weeks of gestation who presented a clinical picture of frank hematuria, disseminated petechiae on the trunk and extremities, asthenia and generalized mucocutaneous pallor. More with hematoma in the right parieto-occipital region, ecchymosis in the oral mucosa and sclera of the right eye. Vital signs are taken, showing blood pressure 130/82 mmHg, heart rate 98lxmin, respiration 22rxmin, temperature 36.6º C, saturation 98%, weight 72.5kg and height 157cm, breast score 0. Peripheral line is placed and Ringer's lactate 1000ml IV 21ml/h, methylprednisone and omeprazole were administered. Laboratory tests, fetal Doppler and weekly ultrasound are performed to evaluate the maternal-fetal status. The test results reported mild anemia, platelets of 2,000mm3/µL. It was decided to transfuse 7 units of platelets and treatment with glucocorticoids was started. The nursing assessment is performed using Marjorie Gordon's functional patterns to identify dysfunctional patterns that compromise maternal-fetal health. And proceed to apply the Nursing Care Process with the objective of providing the optimal care that the patient requires during her hospital stay, and thus meet the objectives proposed in this case study.es_ES

dc.description.abstractEl presente estudio está enfocado en la patología de púrpura trombocitopénica idiopática que afecta el sistema inmunológico y se caracteriza por la disfunción plaquetaria causando el incremento de sufrir hemorragias debido a su tiempo de coagulación. Entre las manifestaciones clínicas más predominante son las hemorragias mucocutáneas y la aparición de petequias en cualquier parte del cuerpo. Esta enfermedad es tratable y con el tratamiento adecuado el paciente puede tener su recuperación total. No obstante, el siguiente estudio de caso clínico está aplicado en paciente femenino de 19 años de edad con 22 semanas de gestación por presentar un cuadro clínico de hematuria franca, petequias diseminadas en tronco y extremidades, astenia y palidez mucocutánea generalizada. Más con hematoma en región parietooccipital derecho, equimosis en la mucosa oral y esclerótica del ojo derecho. Se procede a la toma de constantes vitales donde se evidencia presión arterial 130/82 mmHg, frecuencia cardiaca 98lxmin, respiración 22rxmin, temperatura 36. 6º C, saturación 98%, peso 72.5kg y talla 157cm, score mama 0. Se coloca vía periférica y se administra Lactato de ringer 1000ml IV 21ml/h, metilprednisona y omeprazol. Se realiza exámenes de laboratorio, doppler fetal y ecografía semanal, para poder evaluar el estado materno – fetal. En los resultados de los exámenes se reportó una anemia ligera, plaquetas de 2.000mm3/µL. Se decide trasfundir 7 unidades de plaquetas y se inicia tratamiento con glucocorticoides. Se realiza la valoración de enfermería empleando los patrones funcionales de Marjorie Gordon, para identificar los patrones disfuncionales que comprometen la salud materno-fetal. Y proceder a aplicar el Proceso de Atención de Enfermería con el objetivo de proporcionar los óptimos cuidados que requiere la paciente durante su estancia hospitalaria, y así cumplir con los objetivos propuestos en el presente estudio de caso.es_ES
