dc.descriptionNeonatal abstinence syndrome is the main manifestation in the newborn that appears after drug consumption (Heroin) during pregnancy. Internationally, around 246 million people are affected, that is, 1 in every 20 people between the ages of 15 and 64. The prevalence in Ecuador increases by 5%; during the gestation period, childbirth and postpartum. Today it is known that in the provinces of Guayas and Santa Elena its increase is alarming, according to the SETED (secretariat for comprehensive drug prevention) 2 out of every 3 cities in the coastal region consume more drugs, the highest prevalence is found in: Santa Elena with 5.7%, Durán 4.0%, Guayaquil 8.80% and La Libertad 7.30%. In the present clinical case study, a neonatal patient with a diagnosis of withdrawal syndrome was shown at the University Hospital of Guayaquil, who presented alarm signs such as vomiting, diarrhea, hyperacusis, tachypnea (60 breaths per minute), and tachycardia (182 beats per minute), added to this, there was an alteration in the toxicological tests (positive for opioids), and the findings found in the physical examination (paleness and dehydration). All functional patterns were evaluated, obtaining 4 affected patterns ( elimination and exchange pattern, activity and rest pattern, perceptual cognitive pattern, role-relations pattern) that required prioritized attention in terms of nursing care (fluid management, hyperthermia management and emotional support). After the establishment and application of the nursing care process, a follow-up was carried out, obtaining scores (+2 and +3 on the Likert scale), strict control of their vital signs, and alarm signs (condition and integrity). of skin, membrane and mucous membranes) (hemodynamic status) (neurological reactions to external stimuli), in order to continually evaluate neonatal well-being until the improvement in their health status is confirmed. These nursing activities were carried out throughout the entire hospitalization with the aim of safeguarding the patient's safety and avoiding dangerous states. On her last day, she was discharged by the treating physician who, based on laboratory tests, monitoring of vital signs and completion of pharmacological treatment, provided education to the parent about the consequences of continuing under the influence of opiates, in addition to the staff. Nursing provides emotional and psychological support to avoid such consumption of psychotropic substances and thus avoid new hospitalizations.es_ES

dc.description.abstractEl síndrome de abstinencia neonatal es la manifestación principal en el neonato que aparece después del consumo de drogas (Heroína) durante la gestación. A nivel internacional se efectúa alrededor de 246 millones de personas, es decir 1 de cada 20 personas entre los 15 a 64 años. La prevalencia en Ecuador aumenta un 5%; durante el periodo de gestación, el parto y postparto. Por hoy se sabe que en las provincias del Guayas y Santa Elena su incremento es alarmante, según el SETED (secretaría de prevención integral de drogas) 2 de cada 3 ciudades de la región costera consume más drogas la mayor prevalencia se encuentran en: Santa Elena con un 5,7%, Durán 4,0%, Guayaquil 8.80% y la Libertad 7.30%. En el presente estudio de caso clínico se muestro un paciente neonato con diagnóstico de síndrome de abstinencia en el Hospital Universitario de Guayaquil, el cual presentó signos de alarma como el vómito, la diarrea, la hiperacusia, la taquipnea (60 respiraciones por minuto), y taquicardia (182 latidos por minuto), sumado a esto presento alteración en los exámenes toxicológico (positivo a opioides), y los hallazgos encontrados en la exploración física (pálides y deshidratación), Se evaluaron todos los patrones funcionales, obteniendo 4 patrones afectados (patrón eliminación e intercambio, patrón actividad y reposo, patrón cognitivo perceptual, patrón rol relaciones) que requirieron atención priorizada en cuanto a los cuidados de enfermería (manejo de líquidos, manejo de la hipertermia y apoyo emocional). Posterior al establecimiento y aplicación del proceso de atención de enfermería, se realizó un seguimiento, obteniendo puntuaciones (+2 y +3 en la escala de Likert), se realizó un control estricto de sus constantes vitales, y signos de alarma (estado e integridad de piel, membrana y mucosas) (estado hemodinámico) (reacciones neurológicas a estímulos externos), para así evaluar continuamente el bienestar neonatal hasta constatar la mejora de su estado de salud. Estas actividades de enfermería se llevaron a cabo a lo largo de toda su hospitalización con el objetivo de resguardar la seguridad del paciente y evitar los estados de peligro. En su último día fue dada de alta por el médico tratante quien, basado en los exámenes de laboratorio, monitorización de signos vitales y finalización de tratamiento farmacológico, brinda educación a la progenitora sobre las consecuencias de seguir bajo efectos de los opiáceos, además el personal de enfermería brinda apoyo emocional y psicológico para evitar dicho consumo a la sustancia psicotrópica y de esta manera evitar nuevas hospitalizaciones.es_ES
