Topic: Pyelonephritis is considered a urinary tract infection that commonly begins in the bladder and progresses to one or both kidneys, it can cause anemia complications, as is the case of a 22-year-old patient with 20.5 weeks of pregnancy, who was admitted to the Sacred Heart of Jesus hospital unit. Objectives: In this research work, it was proposed to establish the obstetric behavior in a 22-year-old patient with 20.5 weeks of gestation with pyelonephritis and severe anemia, by collecting clinical history that allows establishing the appropriate diagnosis for pyelonephritis and severe anemia in a pregnant patient. , likewise determine risk factors and outline the appropriate treatment. Methodology: The work that is characterized by being descriptive and investigative of what was observed, clinical examinations, ultrasounds at 20.5 weeks of gestation, history and notes of evolution that demonstrate the patient's situation and confirm the treatment based on ceftriaxone. Which has shown to be very active in conditions of urinary tract infections, ferrous sulfate and folic acid. Conclusion: Through treatment it is possible to control the severe anemia and pyelonephritis of the patient. It is concluded by establishing that pregnant women with abdominal discomfort suspected of pyelonephritis should undergo urine culture tests and, if positive, should be hospitalized and treated with intravenous antibiotics. Adequate education and treatment for pyelonephritis should be provided in order to reduce the burden of this pathology in order to prevent its serious complications.es_ES
