The present clinical case aims to apply in the nursing care process in preschool patients with ventricular septal defect and severe caloric protein malnutrition. During the nursing assessment process, vital signs are taken by observing, a temperature of 35.2°C, 40 breaths per minute, blood pressure of 70/50 mmhg, oxygen saturation of 82% and monitoring of anthropometric measurements resulting in a weight of 6.2 kg. Size of 72 cm, with a BMI of 11.57 indicating low weight, the physical examination shows perioral cyanosis, intercostal strain and difficulty breathing. Doctor orders that a chest X-ray, an echocardiogram, hematological examinations, uroanalysis, coprologic examination be performed immediately. Blood chemistry and IgG-IgM test. After the assessment and the results of the tests, the patient's medical diagnosis was obtained. The present research on this clinical case refers to the pathologies presented by the patient, their causes, clinical signs, risk factors and complications. Therefore, the nursing care process is carried out focusing on altered functional patterns and based on these performing the nursing diagnosis with their interventions by applying the NANDA, NIC and NOC taxonomies. Follow-up and control were performed for the patient. During the patient's stay in the hospital, favorable results were obtained where he was assessed and discharged in good clinical conditions.es_ES
