A case of pyelonephritis is presented in a 15.3 week-old pregnant woman, in the gynecology area of the Hospital Básico de San Miguel, this being one of the most frequent complications of urinary tract infections during the gestational period. In acute pyelonephritis, there are several physiological changes produced by pregnancy that make pregnant women more prone to these infections. Precisely with pregnancy they have been linked to important complications such as: respiratory failure, hydroelectrolytic disorders, chronic renal failure, septic shock, premature rupture of membranes, preterm birth and labour, chorioamnionitis, low birth weight infants, neonatal infections and postpartum fever. Our objective is to apply the nursing care process based on critical thinking in the assessment, diagnosis, planning, execution and evaluation in the care of the pregnant woman and to be able to link this with Marjory Gordon's theory, where we assessed the functional patterns and established the altered patterns which helped us to identify the possible problems related to the patient and to be able to complement the needs that are within our reach and within our competencies, The care plan should prioritizing those that need immediately attention or put the mother and child at risk, thus helping to maintenance reducing the mortality rate of pregnant women in the country.es_ES
