El estudio de caso clínico se trata de un paciente masculino de 26 años de edad que acudió a consulta con su médico por presentar dolor abdominal y mareos constantes. En donde mediante una tomografía computarizada se observo que tenía grasa en el hígado. Se realiza la valoración nutricional del paciente mediante la valoración antropométrica, bioquímica, nutricional y dietética, en referencia a la valoración antropométrica presenta un peso kg 80 y talla de 165 cm lo que refleja en su IMC 30,14 obesidad, en cuanto a sus valores bioquímicos la paciente presenta las enzimas hepáticas alteradas : TGO: 39,10U/L- 5 – 32, TGP: 73.29 U/L- 0 – 54 y GGT: 100,20 U/L- 5 – 85 con un diagnóstico de hígado graso, en lo dietético el paciente presenta un consumo elevado en carbohidratos simples y alimentos procesados . Se recomendó un plan de alimentación en cuanto a las necesidades del paciente con un total de 1900 kcal al día en donde contenga todos los grupos de alimentos y sea rico en fibra, grasas saludables, proteínas y carbohidratos complejos para que tenga un cambio en su alimentación y poder corregir sus malos hábitos alimenticios y mejorar su estado nutricional.

La esteatosis hepática es la acumulación excesiva de grasa en el hígado, esta enfermedad suele presentarse con mayor frecuencia en pacientes que tienen Obesidad. Es una de las enfermedades con alta prevalencia presentándose en un 25 a 30%. En el año 2010 fue la octava causa de muerte en el Ecuador mientras que en el 2011 fue la novena, siendo mas frecuente en los hombres que en las mujeres. Este estudio de caso esta basado en paciente sexo masculino de 22 años de edad que acude a hospital, por presentar síntomas como debilidad, náuseas y color amarillo en la piel. El objetivo de este caso clínico es el proceso de atención nutricional en paciente de sexo masculino de 22 años con diagnóstico de esteatosis hepática, el cual se basa en una dieta de 1377 kcal. La línea de investigación de este caso es salud y bienestar nutricional. Se valoró el estado nutricional del paciente dando como resultado Obesidad de tipo I, se elaboró un plan nutricional para el paciente que constaba de una dieta de 1377 kcal. Se logró que el paciente mejore su calidad de vida, esto se evidencio con su cambio de peso y estado de ánimo.

El presente caso clínico consistió en el proceso de atención nutricional de una paciente que llega por el área de emergencia acompañado de un familiar, el paciente presento un cuadro clínico de tos con flema y presencia de sangre en esputo hace varias semanas, acompañado de pérdida de peso y no poder respirar normalmente. Se solicito Rx estándar de Tórax, Baciloscopia. Cultivo, exámenes bioquímicos adicional prueba de VIH, una vez hecho todos sus exámenes pertinentes se diagnostica Tuberculosis Pulmonar. El medico solicito una interconsulta con el área de nutrición, donde se evaluó su estado nutricional mediante los indicadores antropométrico, bioquímico, clínico y dietético. Obtuvimos su historia clínica y sus valores de los indicadores anteriormente solicitados donde se identificó una desnutrición según su índice de masa corporal (IMC), los exámenes bioquímicos mostraban una Hipertrigliceridemia y el recordatorio 24 horas una ingesta inadecuada de alimentos. El tratamiento nutricional se inició con una introducción de calorías no mayor a las 2.000kcal diarias, sino que fueron 1.800kcal diarias los primeros dos meses para evitar un síndrome de realimentación, también se administró una formula enteral llamada Ensure la cual fue bien tolerada y adaptada a las necesidades calóricas diarias lo cual favoreció al estado nutricional del paciente.

La obesidad es uno de los mayores problemas que preocupa a la salud publica ya que es una enfermedad no transmisible que desencadenas diversas enfermedades. La obesidad mórbida es la forma más grave de sobrepeso. Se le llama mórbida porque el peso excesivo que sufre la persona ocasiona problemas de salud y reduce su esperanza de vida. Este tipo de obesidad está asociada con enfermedades crónicas como las del corazón, diabetes, hipertensión arterial y algunos tipos de cáncer. Se considera que una persona sufre de obesidad mórbida si tiene sobrepeso de por lo menos 50 kilogramos sobre su peso ideal o un IMC (Índice de Masa Corporal) superior a 40 Kg/m2. En este caso clínico se redacta el historial clínico de un paciente de sexo masculino que llega a una cita médica con la cardióloga, y fue ingresado al hospital por problemas cardiacos. El objetivo principal es garantizar que el paciente logre tener una pérdida de peso de manera óptima mediante el proceso de atención nutricional y de esta manera seguir evitando más complicaciones en su salud. En el desarrollo se realiza la valoración nutricional al paciente, empleando los diferentes métodos existentes que son antropométricos, clínicos, dietéticos y bioquímicos, estableciéndose de esta manera un diagnóstico nutricional, para lo cual se estableció una dieta Hipocalórica, Hiposódica, con restricción hídrica de 500cc.
